What is Care Navigation?
Our practice staff have been trained to care navigate our patients to make sure you get the right support at the right time.
As people's health and social care needs are changing, there is a significant need to shift NHS service provision and professional working towards a paradigm of more sustainable, proactive and integrated health and social care. Our care navigation staff can help people get coordinated, person-centered care at the right time and right place.
Where might you be Care Navigated to?
Depending on your needs, here are some examples of where our staff might care navigate you to.
Urgent care:
Walk-in centre
Practice staff might direct patients to the walk-in centre if they need treatment for common illness conditions that are too urgent to wait for a GP appointment but do not need emergency treatment at an A&E department.
Accident and Emergency
Accident and emergency (A&E) is for serious injuries and life-threatening emergencies. These are things like heavy bleeding, choking and seizures. Find out more about when to go to A&E here.
Within the PCN:
Care Co-ordinators
Care Navigation staff may refer you to our practice-based Care Co-ordinators. Our Care Co-ordinators work with specific cohorts of patients and help to co-ordinate and navigate care across the health and care system, helping people make the right connections with the right teams at the right time.
They can support people to become more active in their own health and care and are skilled in assessing people’s changing needs. Care co-ordinators are effective in bringing together multidisciplinary teams to support people’s complex health and care needs.
Practice Care Navigators or GPs will refer you to a Care Co-ordinator.
Emotional Wellbeing Workers
Emotional Wellbeing Workers offer emotional and practical support to patients who have a wide range of clinical and social needs. They assist and coach individuals to ensure they are making healthy and positive choices, have access to the right funds, appropriate housing and are connected to their communities.
Foundry PCN also has a Young Person's Emotional Wellbeing Worker, who provides practical support to people with mental health issues aged 13yrs – 17yrs old.
Practice GPs will refer you to an Emotional Wellbeing Worker.
Social Prescribing Link Workers
Social prescribing link workers connect people to community-based support, including activities and services that meet practical, social, and emotional needs that affect their health and wellbeing. This includes connecting people to statutory services for example housing, financial and welfare advice.
Social prescribing works particularly well for people with low level mental health needs, who feel lonely or isolated, with long term conditions and complex social needs.
Practice Care Navigators or GPs will refer you to a Social Prescribing Link Worker.

PEARS opticians
The NHS has launched a new free eye care service for people with minor eye conditions. Instead of waiting for a GP appointment or attending A&E, you can now make an appointment with your local PEARS qualified optician and be seen within one or two working days.
This is suitable if you have:
- Red eye or eyelids
- Dry eye, gritty and uncomfortable eyes
- Irritation and inflammation of the eye
- Significant recent sticky discharge from the eye or watery eye
- Recently occurring or sudden increase of flashes and floaters
- Painful eye
- Ingrowing eyelashes
- Recent and sudden reduced vision
- Something in your eye
Community Pharmacy Consultation Service
The service connects patients who have a minor illness or need an urgent supply of a medicine with a community pharmacy. The CPCS can deliver a swift, convenient and effective service to meet patients needs.
PCS hub
Remote and face-to-face appointments can be made with a variety of GP practice staff including Doctors, Advance Care Practitioners, Nurses, Nurse Associates, Health Care Assistants, Physiotherapists and, Pharmacists.
Doctor – Will provide assessment, treatment, and advice on a range of health concerns + arrange onward referral if required.
Advanced Care Practitioner – Will provide assessment, treatment, and advice on a range of health concerns + arrange onward referral if required.
Doctor Nurse – Will offer a range of services including long-term condition reviews, health monitoring, wound care, cervical screening, ear irrigation and blood testing.
Nurse Associate – Will offer a range of services including long-term condition reviews, health monitoring, wound care, cervical screening, ear irrigation and blood testing.
Healthcare Assistant – Will offer a range of services including long-term condition reviews, health monitoring, wound care, ear irrigation and blood testing.
Physiotherapist – Can help with muscular-skeletal problems like back pain.
Pharmacist – Can offer advice on medication
Working Win
Working Win is a free service, which aims to support you if you have a physical and/or mental health condition and would like to find meaningful employment or stay in your current role. We also provide you with ongoing in-work support in both your new or existing role. To be eligible for the programme, you must be:
- Over 18 years old.
- Live in Barnsley, Doncaster, Rotherham, or Sheffield.
- Motivated to find or retain meaningful employment.
Our team will work with you to help you find the right job fit and joining this service will not impact on your benefits. You can also opt out of the service at any time, if you feel it is not right for you.
