How To Register

To register with the practice you will need to fill out an application form on the link here: Register with Southey Green Medical Centre - NHS

Overseas Visitors

Please note that overseas visitors are entitled to Free NHS GP services once they register with the practice. Hospital services apart from A&E may attract a charge.

Practice Area

We can register patients from anywhere in Sheffield but cannot offer a home visiting service for anyone who lives outside our catchment area. If your healthcare needs mean you will need home visits it would be better for you to register with a practice closer to home.
